Confidentiality
All information discussed during sessions is held in strict confidence except under the following circumstances:
Office Policy
- You are using your insurance benefits and they require that I disclose your personal health information (PHI) for treatment and payment.
- If I have reason to believe you are a danger to yourself, I must contact your emergency contact person and/or psychiatric emergency services in order to ensure your safety.
- If I have reason to believe you intend to harm a readily identifiable victim, I must take steps to warn and protect that person. This may entail contacting local police departments, your emergency contact person and/or psychiatric emergency services in order to ensure their safety.
- If I have reasonable cause to believe that a child is being subject to abuse, I must report this to the NJ Division of Youth and Family Services.
- If I believe that a vulnerable adult is being subject to abuse, I must contact Adult Protective Services.
Office Policy
- Fees and co-payments are due at the time of service. Payment can be made in cash or online. Please go to my Payment page for more info. Also note that the co-payment quoted by your insurance company may differ from the co-payment after your claim is processed.
- Unpaid balances older than 30 days may be the basis for terminating services and I will make every attempt to assist you in finding alternative affordable treatment. The law allows the referral of unpaid bills to a collection agency or the utilization of small-claims court however this course of action will only be taken if the issue of the delinquent account cannot be resolved otherwise.
- If you must cancel your appointment, you must do so within 24 hours to avoid being charged the full fee for that visit. If you have insurance, they do not pay for missed appointments or appointments cancelled on short notice.
- If you frequently cancel your appointments, you will be asked to terminate your treatment with me. Continuity is essential for psychotherapy to be successful.
- If for any reason your insurance does not pay for services or does not authorize treatment, you will be responsible to pay the full fee for visits.
